Supporting documentation such as Certificates of Capacity, pharmacy receipts, medical reports and relevant correspondence can be emailed separately to: admin@medsreviewed.com.au
Please include the claimant's name in the email subject line.
Privacy notice
MedsReviewed (MEDS REVIEWED PTY LTD, ABN 18 659 006 596) collects personal and health information provided with referrals so we can assess and provide medication review services, communicate with relevant case managers and treating practitioners, and allocate the referral to an appropriate contracted pharmacist.
The referral form is provided through Microsoft Forms. Supporting documents sent to admin@medsreviewed.com.au also form part of the referral information we receive. We use third-party technology and cloud providers (including Microsoft Forms, OneDrive, Wix and email services), and information may be processed or stored in Australia and/or overseas depending on those providers’ infrastructure and configuration.
Relevant information may be provided to the contracted pharmacist assigned to the review and other parties involved in the review where appropriate. If sufficient information is not provided, we may be unable to assess the referral or provide the requested medication review service.
